A detailed breakdown of the fields available in the SaferMe form builder
How to get there
To access the form builder, Admins or Managers of the organisation must begin in the SaferMe web app on the Forms page.
Select Add Form to create a form from scratch, or select Edit Form to update an existing form. The form fields can be found on the Details tab of the form builder.
Please Note: All reports include the time and date of the report's creation, the first and last name of the reporter, and the location of the report.
Short Text Box
- Intended for short sentences
- No maximum word limit
- Text appears on one line
Long Text Box and Description
- Intended for larger bodies of text
- No maximum word limit
- Expands downwards as the user types
- Can press Enter / Return to separate text
Information / Instruction Box
- Used to communicate instructions that appear as display-only text
- Bold text is the best way to emphasise text in this widget
- Italicised, underlined, and strikethrough styles are also available
- Links can be inserted, e.g. link to a web page or document hosted online
- Images hosted online can be inserted into this widget
Numbered List
- For creating an itemised list of steps in the correct order
- Begins with one numbered field visible
- Reporters click + ADD MORE to add to the list
- Steps can be removed by clicking X
Bulleted List
- For creating an itemised list with bullet points
- Begins with one bulleted field visible
- Reporters click + ADD MORE to add to the list
- Points can be removed by clicking X
Slider
- Minimum value is 0
- Maximum value is 10
- Default value can sit anywhere between the min and max value
- Click on Slider Colour to open a colour picker and choose a custom colour
Checkbox
- Multiple options appear in a list for reports to select
- Multiple answers can be selected
- In the .pdf download of a report, checked fields appear with a tick
Drop-down Box
- Multiple options appear in a list for reports to select
- Only one option can be selected as the answer
Radio Button
- Multiple options appear in a list for reports to select
- Only one option can be selected as the answer
Section Break
- Creates a line between fields to separate sections of the form
Photos
- Allows reporters to upload a photo from their device or take a live photo

File Attachments
- Allows reporters to attach a file from their device
- Reporters can add as many files to the one field as they need
Add Hazards from the Risk Register
- Allows for the reporter to embed hazards from the risk register into the form
- The details of the hazards selected can be edited to suit the job at hand
- Edits made on the report will not impact or update the master Risk Register found in the web app
Signature
- Allows workers to add a digital signature to the report
